- Child’s baptismal certificate (if Catholic)
- Child’s record of immunizations
- Most recent physical (dated within the last year)
- Completed online registration form
- Completed student application for 2022-23
- Completed Registration Contract 22-23
- Non-refundable new student enrollment fee of $150
- $250 annual registration fee
